“Questa Cares, Truly Cares!”

WHO WE ARE

We exist to bring integrated home-based lung care through our Individualized Health Coaches (iHCs) who focus on coordinating and delivering care in the patient’s home. iHCs are trained non-medical professionals who desire to help you make positive changes to your health and well-being on daily basis. Specifically, iHCs provide guidance and support to patients as they create and work towards health-related SMART goals. They help you develop healthy eating and exercise habits, manage stress, and improve your overall health and well-being.

By working together with other integrated multi-disciplinary specialist healthcare partners, iHCs help ensure patients receive comprehensive and coordinated care in the comfort of their own homes.

iHC (Individualized Health Coach)

  • An individualized health coach can be very helpful for improving lung care compliance. Compliance refers to a person's willingness and ability to follow their doctor's instructions and recommended treatment plan. When it comes to lung care, compliance is important because following a treatment plan can help manage and improve lung conditions, and prevent complications. A health coach can provide education, support, and motivation to help people with lung conditions better understand and manage their condition, and stay on track with their treatment plan. This can ultimately improve their compliance and help them achieve better health outcomes.

  • iHCs are very helpful for improving communication in lung care. Communication is an important aspect of healthcare and is essential for ensuring that patients with lung conditions receive the best possible care. A health coach can help improve communication in several ways.

First, a health coach can help patients with lung conditions understand their condition and treatment plan. This can involve providing education and support to help patients better understand their condition, medications, and any lifestyle changes they need to make. A health coach can also help patients develop the skills and strategies they need to manage their condition and follow their treatment plan.

Additionally, a health coach can serve as a bridge between patients and other members of the healthcare team. Health coaches can help facilitate communication between patients and their doctors, nurses, and other healthcare providers, and can help ensure that patients receive the information and support they need to manage their condition. By improving communication and coordination between patients and their healthcare team, individualized health coaches can help people with lung conditions receive the best possible care.

  • Comprehensive care refers to the delivery of all necessary healthcare services to patients with lung conditions in a coordinated and integrated manner, which is also known as Pulmonary Rehabilitation. This means that patients receive all of the services they need to manage their condition, including services from specialists such as pulmonologists, respiratory therapists, physical therapists, occupational therapists, smoking cessation specialists, psychologists, social workers, and dietitians.
