We exist to bring integrated home-based lung care through our Individualized Health Coaches (iHCs) who focus on coordinating and delivering care in the patient’s home. iHCs are trained non-medical professionals who desire to help you make positive changes to your health and well-being on daily basis. Specifically, iHCs provide guidance and support to patients as they create and work towards health-related SMART goals. They help you develop healthy eating and exercise habits, manage stress, and improve your overall health and well-being.
By working together with other integrated multi-disciplinary specialist healthcare partners, iHCs help ensure patients receive comprehensive and coordinated care in the comfort of their own homes.
